In 2010, the Department of Health, USA published its new discharge workbook, Ready to Go? Planning the Discharge and Transfer of Patients from Hospital and Intermediate Care,30 which outlined 10 ‘steps’ to ensuring a timely, safe and patient-centred transition from hospital, including:

  • alignment of services to ensure continuity of care
  • efficient systems and processes to support discharge and care transfer

Following such global industry best practices, Synigence Care now works with the Partner Hospital to create an effective patient engagement post discharge program.

Our specialised nurse and client care coordinators follow a diligent calling and sms routine for the first 30 days post discharge to ensure the patient remains engaged. Synigence also offers the Partner Hospital a web based feedback program for all the discharged patients - this provides key trends and statistics on discharge related parameters.
