Per international standards of home health care, Synigence delivery of home healthcare is based on the Plan of Care (POC) focused professional approach.

Synigence team of trained and experienced Senior Nurse Co-ordinators work with Hospital discharge team / Patient consulting physician towards formulating a Detailed POC for the Patient starting from assessment of the Patient’s condition prior to discharge / at time of start of home care.

POC is customized based on each Patient’s medical condition and home health requirement and covers the nursing plan of care, medication schedule, medical equipment need, dietician, physiotherapy and GP visit requirements.

Experienced HDU/Staff Nurse and Attendants are deployed and medical equipment are sourced on hire or to buy based on the Plan of Care needs and requirements.

Qualified, trained and highly experienced Physiotherapist as well as GPs are also provided with relevant experience – visits frequency decided in Plan of Care based on patient’s requirement / condition.

Regular Nursing supervisor visits to the Care Location to ensure Daily chart of care, vitals and medication log are maintained and followed in conjunction with the instruction of the discharging / treating Physician. All patient vitals and progress reports are maintained per detailed health monitoring charts.
